I'm vegan with pretty solid healthy diet. 1 fish oil capsule a day was OK. When I upped it to 2, i broke out in acne across my back, face, and chest.

When I don't take it, it goes away. When i consume them at all now, the breakouts come back.

Actually, fish oil is used as one of the tools to fight acne since it has anti inflammatory properties for all body cells, including the skin. Another tool is a low omega 6 diet, omega 6 is a pro inflammatory oil that we take from all starches, grains and grain oils. It means elimination of all grains, specially wheat and maize, that along with sugars have the power to weaken our immune system.

If you add 30 minutes of daily exercise to the above diet, you will stabilize your hormonal system, the other factor in acne.

In your case, since you are a vegan, I would recommend to switch from the animal omega 3, fish oil, to the vegetable form of omega 3, ALA. You will find ALA, a less active form of omega 3 fat in Flax seeds, flaxseed oil, Chia and some other vegetable omega 3.
